![]() Parents and guardians wishing to establish or continue utilizing the home school program must complete a declaration of intent form and return it within 30 days of the establishment/continuance of the home school program and by September 1 annually, thereafter. It is the responsibility of the parent/guardian to obtain and file a new declaration of intent each year. If you intend to operate a home school program, please submit the home school monthly attendance report to the program specialist at the end of each month during the proposed school year. The blank form may be duplicated. The parent or guardian is responsible for textbooks, curriculum and testing. Please inform the home school office if you wish to discontinue home school for your children.
