Any student wishing to participate in any athletic activity must have the following documentation:

  • A current physical on file
  • Proof of insurance
  • Must pass five or more core classes toward graduation
In this section, you will find a copy of the Physical Evaluation Form, Athletic Participation Form, and Insurance information.
