If the person presenting the student for enrollment is not that student’s parent/guardian or grandparent, the school will require the enrolling person to fully complete the Non-Parental Affidavit until the enrolling person can complete the guardianship process. The enrolling person must stand in loco parentis (i.e., be charged with or have assumed the duties and responsibilities of a parent such as providing food, shelter, clothing and medical care).Please use to request a social security card
If you do not wish for Atlanta Public Schools to disclose directory information from your child’s education records without your prior written consent, you must notify Atlanta Public Schools in writing by completing and submitting the information below by September 1st of each school year.To allow child to be photographed, videotaped and/or voice recorded and for his/her name, image, likeness and voice to be used in APS approved photographs, videos, publications, news media and web pages for special projects or publicity.
COMPULSORY ATTENDANCE NOTICE
Each parent, guardian or person having control or charge of any child between the ages of 6 and 16 are required to enroll and send said child to a public school, private school or any home study program that meets the requirements for public school, private school or any home study program.If a student's residence or contact information changes during the school year, the parent or guardian is required to notify the school. The parent or guardian must complete a change of address form and a new residency affidavit accompanied by new proof of residency documentation. This information must be submitted within fourteen (14) days of any move or change of address. This form should be returned to the Main Office or Registrar’s Office of your child's school. Failure to submit a change of address form will result in the immediate placement of the student in the new school to which he or she is zoned.
