Student Health Forms

Student health forms support key administrative and medical needs. They are used for student enrollment and required health screenings, and they provide the school nursing team with information necessary to create a safe support plan for your child during the school day.

These forms may be required for initial school enrollment, to document health requirements (such as immunizations or screenings), or if your student needs specialized support, such as a 504 Plan, an IEP, or daily medical care at school. Please refer to the information below for more information on each form, when it is needed, and the required signatures.

Commonly Used Student Health Forms

Medical Packets (including medication administration)

Medical packets are official school health forms used to explain your child's medical needs during the school day. They include written medical orders from a licensed healthcare provider and permission from a parent or guardian. This information allows the school nurse or designated school staff to safely give medication or perform medically necessary procedures while your child is at school or participating in school-sponsored activities.

Submit Every School Year

Medical packets are required each school year and are valid for 12 months. Any medical changes require an updated medical packet.

If updated medical orders are not on file, parents or guardians will be responsible for giving medication or completing medical procedures during the school day.

Submit a Medical Packet if Your Child Needs Any of the Following at School:

Prescription medications
Over-the-counter medications (such as ibuprofen or Tylenol)
Emergency medications
Ongoing or specialized health procedures

Form Completion and Signature Requirements

All medical packets and medication administration forms must be completed by a physician (MD or DO) and signed by both the physician and the parent or guardian.

Medical Packet

APS nurses are governed by the Georgia Nurse Practice Act and APS Policy JGCD—Medication: They will only administer medication in accordance with written medical orders signed by a licensed physician, dentist, or podiatrist. APS nurses will not modify any medication dosage based solely on a request or recommendation from a parent or guardian. A parent or guardian seeking a dosage modification must give the nurse an appropriate medical order.

Students carrying medication: Only emergency medications (such as inhalers, epinephrine auto-injectors, and diabetes emergency supplies) may be carried on the student’s person when authorized. A self-administration medication form must be completed and submitted to the school nurse.

All other medications must be stored in the school clinic and administered under supervision by authorized personnel.

Referral for Homebound Services

Homebound services provide temporary school support for students who cannot attend school due to a medical or mental health condition. These services help your child stay connected to learning while they recover at home or in the hospital.

When Homebound Services May Be Approved

Students may qualify for Homebound Services if they are expected to miss 10 or more school days, either consecutively or intermittently (on and off). The absence must be due to a medical or psychiatric condition.

If homebound paperwork is not completed or not approved, your child is expected to attend school as usual.

Who Homebound Services Are For

Homebound services are intended for students whose medical or mental health needs cannot be met in the school setting for a limited period of time.

These services:

Are temporary
Are not a replacement for regular school attendance
Are designed to provide continuity between the classroom and home or hospital while our child heals

Atlanta Public Schools follows Georgia State Guidelines for Hospital/Homebound (HHB) Services for both general education and special education students.

How to Request Homebound Services

Parents may begin the Homebound Services request through their child's:

Form 3300 — Certificate of Vision, Hearing, Dental & Nutrition Screening

Form 3300 is required when a child is enrolling in a Georgia public school for the first time. This form confirms that students have completed the required health screenings, allowing any concerns to be identified and addressed early.

Screenings Required

All four screenings must be completed and documented before Form 3300 can be submitted to the school:

Vision
Hearing
Dental
Nutrition (BMI or BMI percentile)

Who Can Complete the Screenings:

Screenings may be completed by:

Your child's healthcare provider
The local health department
Other licensed professionals approved by the Georgia Department of Public Health, including: Georgia licensed doctor, dentist or dental hygienist, optometrist, audiologist, or dietitian

If Your Child is Transferring Schools

If your child previously attended another Georgia public school and has a completed Form 3300 on file, the previous school should forward the form to the new school.

Who Must Submit Form 3300

Form 3300 is required for all students enrolling in a Georgia public school for the first time.

Form 3300 - Certificate of Vision, Hearing, Dental, & Nutrition Screening

Form 4400 - Certificate of Scoliosis Screening (Grades 6 & 8)

Georgia law (OCGA 20-2-772) requires scoliosis screening for students in 6th and 8th grade. Screening helps identify possible spine concerns early, when treatment is most effective.

What is Scoliosis?

Scoliosis is a sideways curve of the spine that often appears between the ages of 9 and 16. Early detection can help prevent future complications.

How to Complete the Screening

Families may choose one of the following options:

Option 1: Submit Form 4400 - Certificate of Scoliosis Screening

Must be completed by a Georgia-licensed healthcare provider, local health department, or licensed school nurse
Form 4400 must be submitted within the first 90 days of your child's 6th or 8th grade year

Option 2: Participate in the School Screening

Students without a completed Form 4400 on file after 90 days will be screened at school during a scheduled scoliosis screening event

Form 3231 - Certificate of Immunization

Form 3231 is the official Georgia immunization certificate required for students enrolled in Georgia schools and childcare programs. This form verifies that your child has received all state-required vaccines or has an approved exemption on file.

How to Obtain Form 3231

Form 3231 must be obtained from an official Georgia healthcare provider
Sample forms may be viewed online, but copies or sample forms cannot be used for school enrollment.

Who Must Submit Form 3231

Form 3231 is required for all students enrolling in a Georgia public school or childcare program.

Who Can Complete the Form

Form 3231 may be completed by:

A Georgia-licensed physician
A Georgia-licensed advanced practice provider
A Georgia local health department

Form Validity

Form 3231 remains valid until the expiration date listed on the form.
An updated form must be submitted when immunizations expire or when additional vaccines are required.

Exemptions

Medical Exemptions:

Medical exemptions must be documented directly on Form 3231 by a Georgia-licensed healthcare provider.
Medical exemptions must be reviewed and updated annually by the healthcare provider.

Religious Exemptions:

Parents or guardians who object to immunizations based on religious beliefs must submit Form 2208 in place of Form 3231.

Form 2208 — Affidavit of Religious Objection to Immunization

Form 2208 allows parents or guardians to formally document a religious objection to vaccines required for school enrollment in Georgia. This form replaces Form 3231 when a family chooses a religious exemption.

Form Requirements

Form 2208 must be completed by the parent or guardian
The form must be notarized
The completed form must be submitted to the school

Form Validity

Form 2208 does not expire. However, families should notify the school if their circumstances change.

Important Public Health Notice

Students with religious exemptions may be excluded from school during outbreaks of certain communicable diseases, in accordance with public health guidance and state requirements.
