Gifted Eligibility Assessment Referral Form

Thank you for interest in your child's assessment for Gifted Eligibility.  Atlanta Public Schools carries out universal screening each Fall semester to look for all students who meet the current criteria to be referred for Gifted Eligibility Assessment. 

If your child:

1. did not receive a full battery of GATE Gifted Eligibility Assessments througout last school year, AND 
2. is currently in grade K-10, AND
3. possesses a MAP Growth Assessment percentile rank score in Reading or Math, dated within the past two years, at or above these thresholds:

• Grade K at 90th or above
• Grades 1-10 at 85th or above

Your child will automatically be referred for assessment in the upcoming Spring semester. 

Additionally, you may use this form to request to have your child referred for eligibility testing this Spring Semester.  Please fill out each requested field below and submit this request by the September 14, 2026 deadline.  Be sure your child's name and student number match what is on file in the Atlanta Public Schools student information system (no nicknames please). 

Your child will need to meet the criteria for assessment detailed above in bullets 1 through 3.  When submitted, you will receive an on-screen confirmation message of receipt by APS.

In order for evaluation to be conducted, you must provide signed consent below. Please indicate your decision, electronically sign, date, and submit this form.

 

Please contact the school with any additional questions about the assessment process. You will be notified of the results in writing as soon as a gifted eligibility decision is made by the local school Gifted Eligibility Team in May.
