If you have not filled out and submitted the CLINIC FORM for your child/children for 2023-2024 please do so here:

If your child has a diagnosis that requires them to have a “rescue” medication available at school, or if they have a medication that may require a dose during school hours, please remember to have their pediatrician or prescribing doctor complete and email or hand you a copy of the HEALTHCARE MANAGEMENT PLAN a form that can be found in the APS NEW MEDICAL PACKET FOR STUDENTS.
