• Enrollment Form
• Birth Certificate (Original)
• Photo Identification
• Proof of Residency (Lease/Deed)
• Georgia Power (current-within 30 days)
• Affidavit of Residency
• Georgia Certificate of Immunization
• Certificate of Eye, Ear and Dental
• Proof of Legal Guardianship
• Proof of Legal Custody
• Social Security Card (Original)
• 3 additional pieces of mail (if lease/mortgage is NOT in your name)
