The information below is for Midtown Cluster families looking for a quick and easy location to receive a sports physical for the 2025-26 school year. 

 

Good news! Now you have more options!
* DT Howard will be partnering with Virtually Urgent Care to provide sports physicals at DT Howard.
* Families must prepay $30 (Cash App, card, or cash), bring a signed VUH Parental Consent Form, and complete the first two pages (History Section) of the sports physical packet.
* Prepay $30 via Cash App ($VirtuallyUH) ensure your student-athlete gets a sports physical. Type "DTHMS" and your student's name in the "For" section.  
* Only 40 slots are available per session, and sessions must have a minimum of 20 participants.

Exams will be 10:30 am - 2:30 pm
* Friday, May 30th
* Monday, July 21st
* Tuesday, July 22nd
* Thursday, July 31st
