• We are excited to announce our health services with Hazel Health, providing No-Cost Health Services for all APS Families

    With this new service, students will be able to see a Hazel provider right from school or home at no cost to families.

    Caregivers must complete the Hazel consent form before a student can have access to Hazel Health.

    The Consent form can be picked up from Carver Early College in the main office, or it can be downloaded and printed by visiting the Hazel Health web site here:
