    Please print the form below and bring it to Ms. Phillips in the Registrar's Office.

    • There is generally a 24 hour turnaround.
    • It has to be notarized.
    • The form is only good for 30 days, after that the student will have to get another form.

     

    NOTE: 

    If the student is seeking to obtain the license, the student will need to get the ADAPT certificate from Coach Curry.

    The student can only get the certificate if they actually completed Health at Maynard Jackson. 

    The student can email Coach Curry (ccurry@atlanta.k12.ga.us) their name, birth date, and an email address to have the certificate emailed to them.
