- Register Onsite: July 18 - August 5 | 9:00 a.m. - 1:00 p.m.
- Register Online: June 17 - August 5
Required Documents
*The following documents must be provided during onsite registration or uploaded during the application process using a printer, scanner, or cell phone. All documents must be readable (pdf or jpeg).
Enrollment Form
Students new to APS (students entering Kindergarten, coming to APS from other school districts, private schools, or charter schools) should complete an enrollment form
Evidence of Birth
Birth Certificate, Passport, Adoption Record, etc.
(Original Birth Certificate is required for kindergarten)
Photo Identification
Driver’s License, State ID, Passport
Proof of Residency
See Proof of Residency Requirements Sheet*
Certificate of Immunization
Certificate completed by the health department or your private physician
Vision, Hearing & Dental Screening
Certificate completed within the last year on the designated Georgia Form from the health department or your physician and dentist
Social Security Number
Students will be asked for a social security number at the time of enrollment
(Original Social Security Card is required for kindergarten)
Report Card/ Withdrawal
(for students enrolling in grades 1-5)Report card and/or withdrawal form showing promotion or retention status is required for students enrolling outside of APS-(Ms. Alonso will call the school to fax)
