• Child’s Birth Certificate
• Child’s Social Security card 
• Parent/Guardian Identification
• Georgia Certificate of Immunization
• Georgia Certificate of Hearing/Vision/Dental
• Required residency documents (See APS website for details)
• Students Grades 1-5 withdrawal form from previous school or report card

Proof of Residency Requirements

Sarah Smith serves the elementary education needs of North Buckhead, Buckhead Forest and the areas of Pine Hills and Brookhaven that are within the Atlanta city limits. 

• The bona fide address of the student's custodial parent or guardian must fall within the Sarah Smith attendance boundaries for the student to be eligible to attend Sarah Smith Elementary School. 
• To determine if your address falls within the Sarah Smith district, visit the APS School Zone Locator or contact APS Facilities Services Department (404.802.3731). 

Please plan to have ALL the following Proof of Residency documents when you register at the school, but read the complete list for additional information that may be needed.

• Driver’s License of parent or guardian registering the child
• Deed, Mortgage or Lease
• Georgia Power Bill
• Bank Statement, Motor Vehicle Registration or Voter Registration

Please note: Students are required to provide Proof of Residency Documents upon initial enrollment.
